By signing this document, I acknowledge and agree to the following:

Nature of Services

I understand that Solari Wellness provides wellness education, lifestyle guidance, health coaching, functional wellness consulting, laboratory education, and supplement recommendations.

I understand that Solari Wellness services are intended to support overall wellness and health education and are not a substitute for medical care.

No Medical Advice

I understand that Solari Wellness does not diagnose, treat, cure, or prevent disease.

I understand that services provided by Solari Wellness alone do not establish a physician-patient relationship and do not replace the advice, diagnosis, or treatment provided by a licensed physician or other qualified healthcare provider.

I understand that I should continue working with my primary care provider and other healthcare professionals regarding medical concerns.

Laboratory Testing & Educational Review

I understand that laboratory testing may be recommended to gather additional information about my health.

I understand that any discussion, interpretation, or review of laboratory results is provided for educational purposes and should not be interpreted as medical diagnosis or treatment.

Supplement Recommendations

I understand that supplement recommendations are optional and that I am under no obligation to purchase any products recommended by Solari Wellness.

I understand that dietary supplements may not be appropriate for all individuals and that I should consult my healthcare provider regarding potential interactions, allergies, pregnancy, breastfeeding, or medical conditions.

Client Responsibility

I understand that my participation in wellness services is voluntary.

I acknowledge that I am responsible for providing accurate and complete information regarding my health history, medications, supplements, and current health concerns.

I understand that my personal health outcomes depend on many factors and that no specific results can be guaranteed.

Telehealth & Electronic Communication

I understand that services may be provided virtually through secure electronic communication.

I understand that although reasonable safeguards are used, no electronic communication system can guarantee absolute security.

Cancellation Policy

I understand that appointments may be rescheduled or canceled with at least 24 hours’ notice.

Missed appointments or cancellations with less than 24 hours’ notice may be subject to forfeiture of the appointment fee.

Limitation of Liability

I voluntarily assume responsibility for decisions made regarding my health and wellness.

To the fullest extent permitted by law, I agree to release and hold harmless Solari Wellness, its owner, contractors, affiliates, and representatives from any claims, damages, losses, or liabilities arising from my participation in services.
